A case of localized pemphigoid simulating dyshidrosis is described. Only direct immunofluorescence permitted a correct diagnosis. The patient was given 150 gm dapsone, which proved insufficient to clear the lesions to that 10 mg prednisone were added. Treatment was tapered and now, 8 months later, the patient is still in good condition on 75 mg dapsone and 5 mg prednisone.
