Purpose: To describe a case of Acute Syphilitic Posterior Placoid Chorio-retinitis characterized by a peculiar optical coherence tomography (OCT) finding. Methods: The patient underwent comprehensive ophthalmic examination, including ultra-wide field fundus photography, fluorescein angiography, and spectral-domain OCT. Results: This finding was characterized by hyper-reflective lesions located between the retinal pigment epithelium and the outer plexiform layer involving almost the outer nuclear layer. Conclusion: We suggest that this finding could be a sign of outer retina inflammation related to the Syphilis infection and may be helpful in the early diagnosis of ASPCC.
