Purpose To describe the findings of the outer photoreceptor layers using spectral domain optical coherence tomography (SD-OCT), near-infrared and short-wave fundus autofluorescence (NIR-FAF and SW-FAF) in eyes affected by central serous chorioretinopathy (CSC) with serous retinal detachment (SRD), underwent subthreshold diode micropulse laser treatment (STLT) in extrafoveal leakage points. Methods Prospective case series. We studied 28 eyes with acute or chronic CSC during 12 months follow-up. In absence of spontaneous SRD resolution after 6 months of follow-up and persistent active extrafoveal leakage, a STLT was suggested. BCVA assessment was measured on ETDRS charts. SD-OCT analysis was performed by mean of Spectralis-OCT Blue Peack (Heidelberg Engineering, Germany) and NIR-FAF / SW-FAF was analysed with spectralis HRA (Heidelberg Engineering, Germany) Results At baseline, Outer Foveal Layers (OFL) in the detached area had a ‘‘stalactite’’ aspect in all eyes; 13 eyes and 15 eyes showed a homogeneous or irregular pattern, respectively. At final visit, eyes with homogenous stalactite pattern showed improved mean BCVA. Patients evidencing an irregular pattern at baseline showed a substantial stabilization of BCVA during the follow-up. Subgroup analysis evidenced as an early resolution of the neurosensory detachment was associated more frequently with a regular pattern of the OFL, a regular signal on NIR-FAF / SW-FAF and an improved final BCVA. Uneven aspect of the "stalactites" and alteration of the foveal NIR-FAF occurred more frequently in eyes with delayed resolution of the neurosensory detachment and were correlated with lower final BCVA. Conclusions Homogenous ‘‘stalactite’’ pattern of the OFL and regular NIR-FAF / SW-FAF are positive predictor findings for identifying eyes with better functional outcomes in patients with CSC. An early resolution of the neurosensory detachment may be suggested in order to preserve visual function and retinal architecture.
